Primary care is at the heart of communities but too often those who need care most face the greatest barriers. In Leeds, a pioneering partnership between public health, primary care, and the West Yorkshire Integrated Care Board is transforming how local services tackle health inequalities. 

Fairer, Healthier Primary Care in Leeds showcases the innovative tools, strategies, and partnerships developed to embed health equity into everyday general practice. From the creation of a Health Inequalities Template and 5As of Access framework to the GP Trailblazer Fellowship and bespoke PCN infographics, Leeds is empowering frontline teams with practical, data-driven solutions.

The report also highlights the Primary Care Health Inequalities Action Learning Group, a space for primary care staff to learn, share, and drive equity-focused improvements. Through these initiatives, Leeds is demonstrating how collaboration, leadership, and targeted support can make primary care more inclusive, responsive, and equitable.
